LifeWell currently offers services in Massachusetts and Arizona on a self-pay basis. Insurance billing is currently available only for eligible services in Florida. Please review the pricing information on this page for current Massachusetts and Arizona self-pay rates.
A copay is generally a fixed amount your insurance plan may require you to pay for a covered service. A deductible is the amount you may need to pay for covered services before certain insurance benefits begin to pay. Coinsurance is the percentage of the allowed cost you may be responsible for after applicable deductible requirements are met. Your specific responsibility depends on your insurance plan and benefits.
Current self-pay rates are Florida—$300 for an initial psychiatric evaluation and $150 for a medication management follow-up; Massachusetts—$300 for an initial psychiatric evaluation and $175 for a medication management follow-up; Arizona—$325 for an initial psychiatric evaluation and $175 for a medication management follow-up. Massachusetts and Arizona are self-pay only.

For patients using insurance, your specific cost depends on your plan's copay, deductible, and coinsurance, which we may not be able to confirm with certainty before your visit. We recommend verifying your benefits with your insurance provider ahead of time, and our team is happy to help where we can.
